Fire Response Vehicle Identifier Application

This form is for individuals wishing to apply for a Fire Response Vehicle Identifier Sticker for a vehicle used in bushfire response activities within the Shire of Wongan-Ballidu.

Applicants are required to provide their contact details, vehicle information and acknowledge the conditions associated with operating a private vehicle at bushfire incidents.

Please note:

  • A separate application is required for each vehicle.

  • All vehicle operators must have completed Rural Fire Awareness training or higher.

  • Vehicles must meet the safety, equipment and operational requirements outlined in the application.

  • Applicants must read and comply with the Operating Private Equipment at Bushfires guidelines available from DFES.

Submission of this form does not guarantee approval or the issue of a Fire Response Vehicle Identifier.

The personal information you provide in this Fire Response Vehicle Identifier Application is collected for the purpose of assessing and processing your application, verifying vehicle and operator details, issuing and managing Fire Response Vehicle Identifiers, maintaining relevant records, and meeting the Shire's legal and administrative obligations. This information is collected under the Local Government Act 1995 (WA) and any other applicable legislation.

Your personal information will only be used or disclosed for the purpose for which it was collected, or where authorised or required by law. The Shire may share relevant information with the Department of Fire and Emergency Services (DFES), emergency services personnel, government agencies or other authorised organisations where necessary for fire response coordination, administration of the identifier system or compliance with applicable requirements.

Providing the requested information is voluntary; however, if sufficient information is not provided, the Shire may be unable to assess your application or issue a Fire Response Vehicle Identifier.

You may request access to, or correction of, your personal information held by the Shire by contacting the Shire.
